Serenity Now, LLC Release and Indemnity Agreement
I, the Undersigned, hereby acknowledge that I have voluntarily consented to participate in Serenity Now, LLC programming and activities (hereinafter the “Activity”) from Serenity Now, LLC (hereinafter the “Serenity Now”).
In consideration of and as a part of the right to participate in programming at Serenity Now, including but not limited to health, fitness, and wellness activities, by signing this agreement, I hereby release and forever discharge Serenity Now, LLC, and its managers, members, officers, and employees from any and all damage(s), loss(es), claim(s), and cause(s) of action whatsoever, arising out of or in any way connected with my participation with Serenity Now, LLC. I further understand and acknowledge that the Serenity Now assumes no responsibility for any act and/or omission which may cause any damage to any person or their personal property.
I understand that the Activity I am about to engage in voluntarily, entails certain risks, which could result in injury, death, illness, or disease, physical or mental damage to myself or third parties, as well as damages to mine or their personal properties. I am aware of the risks and dangers inherent with health, fitness, and related activities, and I knowingly and willfully assume the risk of injury, including but not limited to injury that might result from the Activity, whether obvious or not obvious. I agree that any bodily injury or disability, death, or loss of personal property and expenses as a result of said injuries, whether caused by my actions or inactions, others’ actions or inactions, the conditions where the Activity takes place, or my own negligence in the Activity is my responsibility. I agree and state that I am fully capable of participating in the Activity, that I am physically able and fully capable to participate in said Activity. I further represent and warrant that I have no pre-existing medical condition that would render my participation in the Activity dangerous to mine or other’s health and safety. I further indemnify Serenity Now for any defects associated with the equipment at Serenity Now whether known or unknown. I agree to immediately report to the nearest Serenity Now official any defective equipment and dangerous condition which I witness.
